Author Questionnaire

• Answer all questions in the questionnaire.  The answers to the questions are considered legally binding, so please answer them carefully and completely.
• The submission letter and cover letter are one and the same. It should indicate the title of the manuscript and must identify the Guarantor and the Corresponding Author, including an address, telephone and facsimile numbers, and e-mail address.
• Insert the abstract in the "Abstract Box, located at the end of the submission process."
• Provide an accurate number of pages, words and illustrative materials.
