Abstract:
In accordance with section 702 of the National Defense Authorization Act for Fiscal Year 2017 (Public Law 114-328), the Defense Health Agency (DHA) recently assumed the authority, direction, and control of all military medical treatment facilities (MTFs). In conjunction with this change, the DHA continues to work diligently on developing policies and procedures that standardize the provision of health care services across the hundreds of MTFs in the Enterprise. Thus far, while numerous DHA Procedural Instructions (DHA-PIs) have been published for various health conditions and health care settings, a large number of Instructions await development. For this presentation, we would like to share our experiences with creating the DHA-PI for suicide risk care with the intent of helping others identify and overcome potential obstacles inherent to the process. We will present the audience with two distinct, but related perspectives on the processes undertaken and the resultant lessons learned: a higher-level, policy-informed perspective and a detail-oriented, clinical perspective. A secondary aim of this presentation is to provide a transparent look at DHA’s internal processes with respect to health care standardization efforts.
DHA-PI development processes to be discussed include the formation of a Work Group with representatives from each of the Armed Forces, including the Coast Guard; a systematized review of more than 100 policies, reports, and guidelines; selection of the Instruction’s fundamental guiding document; basics for drafting the Instruction and the solicitation and incorporation of feedback from the Services; consultation and coordination with Department of Veterans Affairs (VA) and GENESIS (new DoD/VA electronic health record system) subject matter experts; and coordination with the DHA’s Behavioral Health Clinical Community. The majority of lessons learned that we will discuss correspond to the categories of harmonization of procedures between Services, across health care settings, and between systems of connected care; finding balance between provider-focused concerns and standardization concerns; and achieving compliance with all relevant accrediting body requirements and DoD issuances. We look forward to exchanging ideas with the audience and deepening the discussion during a question and answer period at the end of the presentation.
